UCR Employee 2020-21 Flu Vaccine Requirement

UCR Faculty and Staff,

To support the health and well-being of UC students, faculty, and staff and our communities, the University of California – in consultation with UC Health leadership – has issued a systemwide executive order requiring all members of the UC community to receive an influenza immunization before Nov. 1, 2020More information about the flu vaccination requirement can be found hereAll faculty and staff will be expected to confirm receipt of the flu vaccination for 2020-21 using the Employee Flu Vaccination Requirement Form. Please review the UCR Flu Vaccine standard operating procedure. 

 If you require a medical exemption, you must download the medical exemption form below and have it completed by your healthcare provider. Any request for a medical exemption, or a request for a disability or religious accommodation, will be routed to the appropriate office for review and follow-up. If you already have a medical accommodation document from your physician, you will be required to upload it as part of this process. Information on reasonable accommodations can be found on the Human Resources website.

 Each employee will receive an email with a personalized link to complete the flu vaccine requirement form. Please complete the electronic form to report receiving your flu vaccine or to upload a request for an accommodation or exemption. Upon completion, you will receive a confirmation email.  

 If you have not been vaccinated and would like to schedule an appointment, UCR Health will be scheduling weekly drive-thru flu vaccine clinics each Friday through the month of October on campus at Parking Lot 25, in front of the Student Recreation Center-North. If you would like to request a flu vaccine appointment on campus, you can make an appointment here

 NOTE: UCR Health and Student Health Services employees working in clinics will be required to upload proof of vaccination. This includes physicians, medical assistants, residents, medical students, fellows, clinic management, and anyone else who interacts with patients in clinics.
