Dear Campus Community,
The UC Office of the President recently shared the details of a proposed policy on COVID-19 vaccination for employees and students at all University of California campuses. While the policy has not yet taken effect, we are preparing for the likelihood of the policy’s adoption.
We know that thousands of UCR employees have already been vaccinated, many at UCR Health clinics held on campus and others at county clinics and private healthcare providers.
UCR is beginning the process of tracking the university’s rate of vaccination, which will be required as part of the proposed UC policy.
The following link is to the new COVID-19 Vaccine Tracking Form, which allows all UCR employees to confidentially report that that they have been fully vaccinated for COVID-19.

While completing the form is entirely voluntary at this time, I strongly encourage any employee who has completed their vaccination series (either through two doses of the Pfizer/Moderna vaccines or one of the Johnson & Johnson vaccine) to fill it out as soon as possible.
If you haven’t yet completed the vaccine series, I encourage you to fill out the form once you have received the vaccine.
The Chancellor’s executive team participated in a pilot of the form, which also includes uploading a photo of a COVID-19 Vaccination Record Card, earlier this month.
Any employee who completes the COVID-19 Vaccine Tracking Form will receive a new UCR R’Card, which normally costs $30 to order. The R’Card can be used as payment at campus stores and restaurants and offers several other benefits such as UCR Library privileges, educator discounts, and access to campus facilities.
Submitting the vaccination record will mean you will not have to so later, and will allow the university to better know the percentage of employees who have been fully vaccinated – which is essential toward further reducing restrictions on campus and ensuring the health and safety of our campus community.
