Submitting proof of COVID-19 booster to student portal

We write today to stress the importance of the previously communicated requirement that all eligible students must obtain a COVID-19 vaccination booster by January 31, 2022. Information on who is eligible for a booster can be found here.
Per the UC Office of the President policy, all students must be vaccinated for COVID-19 for the 2021-2022 academic year, which includes staying current with boosters.
Upload your proof of completion to the patient portal by January 31, 2022. A video tutorial is available online. If you have any questions or need further advisement, please submit questions through the patient portal. Choose Messages, create New Message and choose the COVID-19 Mandate Staff.
COVID-19 vaccines are available at Student Health Services, adjacent to Lot 15.  To schedule your COVID-19 vaccine appointment, please call Student Health Services at 951-827-3031.  Please note that your vaccine will be free regardless of insurance.
The vaccination policy includes a narrow range of exceptions for individual medical reasons or religious beliefs. If you have already submitted your exception, you do not need to submit this information again. However, anyone who has been approved for vaccine exemption must continue to comply with mandated testing and health and safety protocols.
